About The Position
Ennoble Care is seeking a Full-time Intake Nurse to work on a hybrid schedule and support our Freehold, NJ market. Ennoble Care is a mobile primary care, palliative care, and hospice service provider with patients in multiple states. They offer a variety of programs including remote patient monitoring, behavioral health management, and chronic care management. The company values honoring patients and providing high-quality care in a strong team-oriented environment.
Requirements
- High school graduate or GED
- LPN license
- Must be computer literate and able maintain simple records
- Must be able to communicate effectively orally and in writing
- Must have at least 3 years of Customer service experience
- Must have at least 3 years of EMR System experience
Nice To Haves
- Experience in a Home Health Care or Hospice setting preferred
Responsibilities
- Answering phones, receives all referral and intake information.
- Complete referral/intake forms and route them appropriately for admission approval.
- Assist in the scheduling of appropriately skilled person to fill the case, inputs scheduling information on the computer.
- Check common working files for Medicare Coverage and verification of benefit period.
- Perform other duties as directed.
