Responsible for being able to perform all aspects of the hospital’s assessment and referral process, including efficient and effective handling of inquiry calls, knowledge of and communication with referral sources; providing hospital and program information to individuals, agencies, facilities, and professionals; making outside referrals, when appropriate; assessing and triaging an individual’s mental health and/or chemical dependency status; making patient safety and level of care recommendations to the physicians(s); identifying insurance benefits, pre-certification of patient care; completing all admission requirements; communicating patient safety concerns and pertinent patient information to unit nursing staff and documenting all necessary information. Requirements

  • Master Degree
  • 1 to 3 years of experience

Responsibilities

  • Perform all aspects of the hospital’s assessment and referral process.
  • Handle inquiry calls efficiently and effectively.
  • Communicate with referral sources.
  • Provide hospital and program information to individuals, agencies, facilities, and professionals.
  • Make outside referrals when appropriate.
  • Assess and triage an individual’s mental health and/or chemical dependency status.
  • Make patient safety and level of care recommendations to the physician(s).
  • Identify insurance benefits and pre-certify patient care.
  • Complete all admission requirements.
  • Communicate patient safety concerns and pertinent patient information to unit nursing staff.
  • Document all necessary information.
  • Know and understand the assessment and referral call system procedure.
  • Follow the Intake call sheet to resolution.
  • Know the Hospital services and be able to answer general service information questions, including admission and exclusion criteria.
  • Complete assessment and evaluation of presenting patients.
  • Inform patients/families of patient rights, responsibilities, and pertinent rules and regulations prior to admission.
  • Follow EMTALA regulations and identify high risk individuals presenting for emergency care.
  • Effectively communicate information to physician for appropriate orders for services.
  • Initiate verification of insurance coverage to determine viability of insurance for individuals seeking admission.
  • Perform other duties as assigned/required by this position.
