About The Position

The Intake Coordinator ensures daily communication to the team and completes clinical administrative duties for the organization of the interdisciplinary team at AmeriCare Home Health Care.

Requirements

  • Associate degree preferred; or one to two years related experience and/or training; or equivalent combination of education and experience preferred.
  • Medical office experience preferred.
  • Knowledge of basic typing skills, data entry and word processing software.
  • Database software and general knowledge of e-mail is preferred.
  • Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents.
  • Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community.

Responsibilities

  • Maintains positive and effective communication with all employees, referral sources, and customers.
  • Follows the Avow Compliance and Ethics Plan.
  • Ensures confidentiality of all clients, family, and employee information in accordance with HIPAA and company policies.
  • Processes new referral information into the electronic medical record (EMR) system in a timely manner.
  • Manages and processes all incoming referrals and intake data received via platforms such as Focura, Epic, Pointcare, and other communication channels.
  • Enters all referral information into the WellSky ERM accurately, ensuring proper documentation and coding.
  • Sorts, separates, and labels incoming referral documentation, including H&P (History & Physical), and Face-to-Face (F2F) evaluations, and organizes them into appropriate patient files.
  • Conducts welcome calls to all new referrals to verify demographic data, gather access instructions (e.g., door/gate codes), and update information in WellSky ERM accordingly.
  • Maintains and monitors adequate inventory of Start of Care (SOC) packets; distributes to assessing clinicians as required.
  • Gains and maintains proficiency in Forcura, WellSky, and Adobe Sign to manage digital documentation and referral workflows efficiently.
  • Prepares, sends, and distributes outgoing and incoming faxes as needed.
  • Monitors inventory and reorders SOC folders to ensure supply meets demand.
  • Sets up and orders wound vac for clients with orders.
  • PT/INR oversight to include faxing results to MDs daily.
  • Telehealth Oversight / trend monitoring.
  • Provides support to the Scheduler and other team members as needed to ensure smooth operations.
  • Adheres to all company policies, including compliance and ethics program guidelines.
  • Performs all other duties as assigned to support the Intake and Clinical Operations team.
