About The Position

Catholic Charities Brooklyn and Queens, with over 125 years of service, offers 160-plus programs and services across Brooklyn and Queens for various age groups and individuals facing mental health challenges. Through programs like Benson Ridge and Southwest Queens Senior Case Management, they provide case management and homecare services to homebound elderly individuals. These services include care management plans, homecare assessments, and connections to services like Home Delivered Meals and Friendly Visiting. The primary goal of the Case Management Program is to offer essential support to enable frail seniors to live safely within their homes and communities. The Intake Coordinator is responsible for conducting a thorough review of Case Management services and screening potential older adult callers to determine their eligibility for services. If a caller is found eligible, the intake worker will input essential information to begin the intake process and enroll the client in the NYC Aging database, after obtaining the caller's consent. The Intake Coordinator must also be capable of explaining case management services to other community callers.

Requirements

  • Bachelor’s Degree in Human Services or related services from an accredited college or university.
  • Good verbal and written command of the English Language.
  • Proficient in computer software applications, Excel, and Microsoft Office.
  • Within 6 months of hire date learn the NYC Aging client software system for data entry of client files
  • Ability to lift up to 10lbs.
  • Must be able to travel to conferences and other locations for trainings and meetings as needed

Nice To Haves

  • Knowledge of entitlements for older adults preferred.
  • Bilingual preferred.

Responsibilities

  • Perform intake activities, which include assessing eligibility, preliminary evaluation of the presenting problem, initiating services as needed and providing information and referral services.
  • Maintain a current resource book of available providers and community resources for callers
  • Providers callers with information on entitlements and community-based programs and resources
  • Provide emergency coverage for Home Delivered Meal clients when the client fails to answer the door, when needed.
  • Maintain up to date: case notes, waiting lists for clients and the timely submission of monthly statistics and reports as required by the agency and the funding source.
  • Attend trainings including yearly training hours required by NYC Aging and other agency trainings/meetings.
  • Utilize the Vive data system for client information and screen for benefits with UNIFORM system.
