About The Position

Catholic Charities Brooklyn and Queens, with over 125 years of service, offers numerous programs and services across Brooklyn and Queens. Through initiatives like the Benson Ridge and Southwest Queens Senior Case Management Programs, they provide case management and homecare services to homebound elderly individuals. These services include developing care management plans, conducting homecare assessments, and connecting clients with resources like the Home Delivered Meals and Friendly Visiting programs. The primary objective of the Case Management Program is to offer essential support, enabling frail seniors to live safely within their homes and communities.

Requirements

  • Bachelor’s Degree in Human Services or related services from an accredited college or university.
  • Knowledge of entitlements for older adults preferred.
  • Good verbal and written command of the English Language.
  • Proficient in computer software applications, Excel, and Microsoft Office.
  • Ability to lift up to 10lbs.
  • Must be able to travel to conferences and other locations for trainings and meetings as needed.

Nice To Haves

  • Bilingual preferred.
  • Within 6 months of hire date learn the NYC Aging client software system for data entry of client files.

Responsibilities

  • Provide a comprehensive review of Case Management services and screen potential older adult callers for eligibility.
  • Input critical information to initiate the intake process and case enrollment in the NYC Aging database upon obtaining caller consent.
  • Communicate case management services to other callers from the community.
  • Perform intake activities, including assessing eligibility, preliminary evaluation of the presenting problem, initiating services as needed, and providing information and referral services.
  • Maintain a current resource book of available providers and community resources for callers.
  • Provide callers with information on entitlements and community-based programs and resources.
  • Provide emergency coverage for Home Delivered Meal clients when the client fails to answer the door, when needed.
  • Maintain up-to-date case notes, waiting lists for clients, and ensure timely submission of monthly statistics and reports as required by the agency and the funding source.
  • Attend trainings, including yearly training hours required by NYC Aging and other agency trainings/meetings.
  • Utilize the Vive data system for client information and screen for benefits with the UNIFORM system.
