About The Position

The Intake Coordinator will directly educate a prospective patient through the intake process leading to enrollment with Always Compassionate Home Care. This person will work closely with referral sources, account managers, clinical team, and provider relations to ensure all tasks are efficiently completed associated with the enrollment process. The coordinator's direct job function would allow for a patient to receive the proper care they need and confirm all information about said patient has been recorded correctly.

Requirements

  • Working knowledge of NYS DOH Health Care operations.
  • Excellent organizational and time-management skills.
  • At least 3-5 years of experience in a Home Care setting.
  • Strong communication and interpersonal skills.
  • Attention to detail with problem solving skills.
  • Strong customer service background.
  • Ability to use time efficiently.
  • Ability to work in a fast-paced environment and demonstrated ability to multi-task.
  • Technically savvy - proficient in use of smartphones, tablets, laptops, and other devices.
  • Exceptional communication skills - demonstrated ability to communicate and follow-up in a timely manner.
  • Bachelor’s degree required.

Responsibilities

  • Educate a prospective patient through the intake process leading to enrollment with Always Compassionate Home Care.
  • Work closely with referral sources, account managers, clinical team, and provider relations to ensure all tasks are efficiently completed associated with the enrollment process.
  • Ensure a patient receives the proper care they need and confirm all information about said patient has been recorded correctly.
  • Obtain initial authorization for care from respected parties- Pharmacies or insurance companies.
  • Refer to scheduling department need for coverage.
  • Answer phones from referral sources for intakes.
  • Report back to referral source status of start of care.
  • Document and report to Supervisor when there is a change in client status, change in service requirement, change in employee assigned to case and any unsatisfactory performance.
