About The Position

The Intake Coordinator coordinates patient referrals and serves as a resource for referral sources. The role collaborates with branch and sales teams to ensure referrals are adequately coordinated and patients transition to services. It also requires applying and demonstrating knowledge of regulatory guidelines and governing processes. The compensation range is $22.00–$27.00 per hour, based on years of experience. Driving requirements do not apply to employees located in Oregon unless they have a clinical license.

Requirements

High school diploma or equivalent.
At least two years of experience in the health care industry and one year of experience in home health or pediatrics.
Demonstrated customer service experience.
Demonstrated knowledge of Medicare guidelines governing home health or hospice.
Basic demonstrated technology skills, including operation of a mobile device.
Valid state driver's license.
Maintain automobile liability insurance as required by law.
Maintain dependable transportation in good working condition.
Ability to safely drive an automobile in all types of weather conditions. Driving requirements do not apply to employees located in Oregon unless they have a clinical license.

Nice To Haves

Graduate of an approved school of professional nursing and licensed in the state where currently practicing; may be a registered nurse or licensed professional/vocational nurse.
Experience with intake responsibilities and working with a multi-location company.
