About The Position

The Intake Coordinator works as part of a multidisciplinary team serving community-based patients within the Mosaic Primary Care Network. The Intake Coordinator reports directly to the Supervisor, Mental Health and plays a key role in creating a seamless and supportive experience for patients being referred to Mosaic for mental health services. Serving as the first point of contact, this position guides patients through an established clinical intake process, gathers essential information, and addresses questions to ensure clarity and understanding. The role is responsible for collecting and documenting patient data while maintaining strict confidentiality and compliance with organizational and regulatory policy. The Intake Coordinator will facilitate entry into MPCN mental health services – including appointment scheduling – and will coordinate referrals to community mental health and other resources as appropriate. The Intake Coordinator supports collaborative working relationships with external primary care providers and the MPCN multidisciplinary team and participates in ongoing quality improvement activities, program development, and evaluations projects to support the continued refinement and delivery of high-quality clinical care.

Requirements

  • Completion of a Social Work Diploma (2-year) from an accredited institution, or a bachelor’s degree in a mental health or related human services field which may include Addictions Counselling/Counselling or Psychology.
  • Current registration with an appropriate professional association or regulatory body, as applicable. Active registration must be maintained.
  • Completed certificates in Mental Health First Aid and/or risk identification and de-escalation training (e.g. Suicide Prevention, Risk Assessment & Management (SPRAM), Applied Suicide Intervention Skills Training (ASIST), Crisis Prevention & Intervention (CPI))
  • Minimum of 2 years of direct clinical experience working with adult patients with mental health concerns in a mental health focused setting.
  • Demonstrates strong critical thinking skills, including contextual awareness, clinical judgement, ethical reasoning, and problem-solving skills.
  • Skills in crisis intervention and self-harm risk management strategies.
  • Awareness and sensitivity in working with diverse and vulnerable populations, including” Cultural and ethnic minorities, LGBTQS+ individuals, persons with disabilities, and/or chronic illness, refugees, older adults, and economically disadvantaged individuals.
  • Working knowledge of local community and mental health services, crisis support resources, and associated referral processes.
  • Able to work collaboratively as a member of a multidisciplinary team, and with Primary Care Providers.
  • Strong verbal, interpersonal and written communication skills.
  • Strong time management and organization skills.
  • Self-directed, flexible, and displays initiative.

Responsibilities

  • Conducts clinical intake appointments with patients referred to the Mosaic PCN for mental health services to determine eligibility and coordinate appropriate supports.
  • Under supervision of the Supervisor, Mental Health/Clinical Supervisor, performs psychosocial and risk assessments, including routine screening for suicide and self-harm, intimate partner violence/elder abuse.
  • Under the guidance of the Supervisor, Mental Health/Clinical Supervisor, coordinates appropriate crisis support resources for patients experiencing imminent risk.
  • Uses validated brief screening questionnaires and instruments, as appropriate, to guide intake assessment and document current symptoms (e.g. PHQ-9, GAD-7).
  • Appropriately triages and conducts scheduling on initial appointments with Mosaic PCN Mental Health therapists, while coordinating any appropriate interim or complimentary referrals.
  • Works in collaboration with ineligible referred patients to coordinate appropriate referral to community mental health services to support treatment of complex or urgent mental health concerns.
  • Works in collaboration with ineligible referred patients to coordinate appropriate referrals to Mosaic PCN programs where appropriate.
  • Provides education to eligible patients around Mosaic PCN Mental Health program services to clarify expectations and support readiness for 1:1 services.
  • Actively monitors MHT program wait times and communicates trends with the Supervisor, Mental Health to support schedule optimization and timely patient access.
  • Demonstrates the ability to work as a member of a multidisciplinary health team and with Primary Care Providers (PCPs).
  • Actively participates in group case consultation and one-to-one case consultation with colleagues, Supervisor, Mental Health and Clinical Supervisor as needed.
  • Supports the orientation and development of team members through consultation, mentorship, and facilitating opportunities for new team members to observe clinical practice.
  • Documents all phases of patient care in the appropriate Electronic Medical Record (EMR) to maintain accurate and timely records.
  • Participates in ongoing program evaluation through the diligent recording of clinical interactions and patient data in Mosaic PCN evaluation systems.
  • Participates in ongoing quality improvement initiatives within Mosaic PCN and the Mental Health Team.
  • Participates occasionally in community activities within the Mosaic PCN catchment area, including health fairs, conferences, and other community events to promote Mosaic programs and services.
  • Identifies and works to achieve own professional development needs which support goals set in performance reviews.
  • Seeks mentorship to achieve full potential in professional development and provide optimal patient care.
  • Other responsibilities as required.
