About The Position

The Intake Coordinator works closely with referral sources, patients, and families to ensure a smooth and timely admissions process. This position combines customer service, intake coordination, and sales support, while giving you exposure to referral source relationships and sales strategy. This role is designed as the first step in our hospice sales career path. Successful Intake Coordinators have the opportunity to transition into positions in Business Development or Operations.

Nice To Haves

  • Bachelor’s degree preferred (business, healthcare, communications, or related field) or equivalent experience.
  • Customer service or sales experience a plus, but not required—training provided.

Responsibilities

  • Serve as the first point of contact for inbound referrals via phone, fax, or email.
  • Communicate with referral sources (hospitals, physician offices, skilled nursing facilities) to gather necessary information.
  • Provide education on hospice services to patients, families, and providers (training provided).
  • Enter accurate referral and patient information into the EMR system.
  • Coordinate with admissions and clinical teams to ensure timely scheduling of services.
  • Support the sales/marketing team by maintaining referral logs, preparing reports, and following up on pending referrals.
  • Participate in training, ride-alongs, and mentoring sessions with business development team.
  • Develop professional relationship-building skills with the long-term goal of advancing into external sales.
