About The Position

The Weekend Intake Coordinator will process and finalize new patient referrals in our Homecare Homebase electronic medical record system. This role involves completing patient welcome calls, documenting coordination notes, and following up on pending and hold patients by contacting hospitals and rehabilitation facilities for updates. The coordinator will also complete pending and hold reports for assigned districts, work closely with the scheduling team to ensure a smooth transition into home health care, and provide clerical support to the Nursing Supervisor. Additionally, the role requires answering incoming calls, determining the appropriate response, and connecting callers with the correct resource. This is a part-time, in-office position located in Stuart, FL, requiring work on Saturdays and Sundays from 8:00 AM to 5:00 PM with a 1-hour lunch break.
