About The Position

Center of Illinois Home Health Care, Inc. is seeking a detail-oriented and customer-focused Home Health Intake Coordinator to join our growing team. The Intake Coordinator plays a vital role in ensuring a smooth and efficient patient admission process by managing referrals, insurance verification, authorizations, scheduling coordination, and communication with referral sources, patients, and healthcare providers. The Home Health Intake Coordinator is responsible for coordinating all aspects of the patient intake process, including receiving referrals, verifying insurance benefits, obtaining authorizations, maintaining referral relationships, and assisting with patient admissions. This position serves as a key liaison between referral sources, patients, clinicians, and agency leadership.

Requirements

  • High school diploma or GED required.
  • Proficiency with computer systems, Microsoft Office, and electronic medical records.
  • Ability to work independently while managing multiple priorities.

Nice To Haves

  • Minimum of 2 years of home health experience preferred.
  • Experience with home health intake, referrals, insurance verification, or healthcare administration preferred.
  • Strong organizational and multitasking skills.
  • Excellent verbal and written communication abilities.
  • Knowledge of Medicare, Medicaid, and commercial insurance authorization processes is preferred.

Responsibilities

  • Manage daily patient referral and intake operations.
  • Verify insurance eligibility, benefits, and authorizations.
  • Coordinate admissions and scheduling of clinical staff.
  • Maintain accurate intake records and documentation.
  • Build and maintain positive relationships with referral sources.
  • Ensure compliance with agency policies and regulatory requirements.
  • Assist referral sources with information regarding available services and community resources.
  • Track physician orders and obtain signed documentation as required.
  • Communicate professionally with patients, families, physicians, referral partners, and team members.
  • Answer incoming phone calls and direct inquiries appropriately.
  • Maintain patient confidentiality and HIPAA compliance.
  • Participate in quality improvement initiatives and agency training programs.
  • Provide administrative support to agency leadership as needed.
