About The Position

The Alzheimer Society of Peterborough, Kawartha Lakes, Northumberland and Haliburton is seeking a compassionate and detail-oriented professional to join our team as an Intake Coordinator. In this role, you will be the first point of connection for individuals and families seeking support. You will play a key role in creating a welcoming, respectful, and person-centred experience, helping people feel heard, informed, and supported from their very first interaction with our organization. As the Intake Coordinator, you will receive referrals and connect with care partners, and individuals living with dementia to gather important information to help guide their journey with our services. You will complete components of the First Link Standardized Assessment and support data collection to advance health equity. You will work closely with the Counselling and Navigation team to ensure individuals are connected to the right supports at the right time.

Requirements

  • Diploma or degree in Social Work, Social Services, Geriatric Care, or a related field (or equivalent experience)
  • Supporting individuals living with dementia and their care partners
  • Assessment, care planning, or service coordination
  • Working in healthcare, human services, or inter-professional environments
  • Knowledge of dementia care and person-centred practices
  • Excellent interpersonal and communication skills
  • Strong organizational and time-management abilities
  • Cultural sensitivity and awareness of diversity
  • Knowledge of community, health, and social service systems in the region
  • Proficiency in Microsoft Office and client databases
  • Valid G Class driver’s license and access to a reliable, insured vehicle
  • Vulnerable Sector Police Check (within the last 2 months)

Nice To Haves

  • Preference given to regulated professionals in good standing with an Ontario regulatory body
  • Membership in a relevant regulatory body is an asset

Responsibilities

  • Respond to email, phone, and in-person inquiries for support and services
  • Provide information about Alzheimer Society programs and services
  • Receive and process professional and self-referrals
  • Complete portions of the First Link Standardized Assessment
  • Gather information about individuals’ circumstances and support needs in a person-centred, non-judgemental manner
  • Collect information to support health equity initiatives
  • Review privacy, confidentiality, and consent for service with new clients
  • Coordinate completion of consent and communication documentation
  • Provide welcome/information packages and follow up within established timelines
  • Accurately document client interactions and information in the client database
  • Collaborate with the Counselling and Navigation team to ensure appropriate service connections
  • Occasionally provide coverage for social recreation programs, as needed
