About The Position

We are seeking an energetic and results driven Intake Coordinator to join our team. The Intake Coordinator works closely with referral sources, patients, and families to ensure a smooth and timely admissions process. This position combines customer service, intake coordination, and sales support, while giving you exposure to referral source relationships and sales strategy.

Requirements

  • Bachelor’s degree preferred (business, healthcare, communications, or related field) or equivalent experience.
  • Customer service or sales experience a plus, but not required—training provided.
  • Strong communication, interpersonal, and organizational skills.
  • Compassionate, professional, and comfortable working in sensitive healthcare situations.
  • Motivated, coachable, and eager to grow into a sales career.

Responsibilities

  • Serve as the first point of contact for inbound referrals via phone, fax, or email.
  • Communicate with referral sources (hospitals, physician offices, skilled nursing facilities) to gather necessary information.
  • Provide education on hospice services to patients, families, and providers (training provided).
  • Enter accurate referral and patient information into the EMR system.
  • Coordinate with admissions and clinical teams to ensure timely scheduling of services.
  • Support the sales/marketing team by maintaining referral logs, preparing reports, and following up on pending referrals.
  • Participate in training, ride-alongs, and mentoring sessions with business development team.
  • Develop professional relationship-building skills with the long-term goal of advancing into external sales.
