The Intake Coordinator provides a single point of entry for in-patient intake. Duties include phone triage, addressing requests for services, performing patient call backs, and documenting all call activities. The role involves answering inquiry calls about hospital services and bed availability, performing pre-registration, collecting pertinent data, verifying insurance eligibility, and coordinating with insurance companies. The Intake Coordinator facilitates and coordinates assessments and/or intake to facility services when appropriate, and maintains communication between the organization and patient referrals and family.
