About The Position

PeopleCare Health Services is looking for an Intake Coordinator to help create a smooth, professional experience for new clients and families from the moment a referral is received through start of care. This role is a great fit for someone who enjoys working with people, thrives in a fast-paced environment, pays close attention to details, and takes ownership of getting things done. As an Intake Coordinator, you will manage intake activity from referral through start-of-care readiness. You’ll work closely with clients, families, referral partners, caregivers, scheduling teams, and operations staff to make sure every new client receives timely and accurate support.

  • Strong follow-through, accountability, and ownership of assigned work.
  • Proficiency with Microsoft Office and the ability to learn company technology and intake systems.
  • The ability to communicate professionally with clients, families, referral partners, case managers, caregivers, scheduling teams, office staff, and other third parties.
  • The ability to work independently while staying connected and aligned with the broader team.
  • Comfort supporting virtual intake and assessment activities.

Responsibilities

  • Receive incoming client referrals by phone, email, and fax and accurately enter referral information into company systems.
  • Conduct initial referral calls and help determine client service needs.
  • Gather required background information and follow up on missing or incomplete documentation.
  • Coordinate and schedule initial intake assessments.
  • Prepare intake materials and review new admission paperwork for accuracy and completeness.
  • Support virtual intake and assessment activities, including technology setup, documentation, and planning.
  • Communicate with scheduling and operations teams regarding staffing needs, timelines, client risks, and start-of-care requirements.
  • Track open referrals, pending documents, assessment dates, follow-up items, and handoffs through completion.
  • Maintain accurate referral source, intake, and business development tracking information.
