About The Position

The PACE Intake and Community Outreach Specialist is responsible for screening potential new enrollments, evaluation and coordination of new enrollees into the PACE Program. Establishing and maintaining relationships with referral sources, Oklahoma Health Care Authority (OHCA) and Department of Human Services (DHS). Responsible for reporting inquiry, referral and enrollment information and statistics. Maintains a referral tracking system and routinely reports to Intake Manager. Educating the Tulsa community, generating leads for LIFE PACE, conducting sales/education/marketing presentations, creating and maintaining relationships with key referral sources and supporting budgeted census goals for LIFE PACE, Inc.

Requirements

  • Holds and maintains an Oklahoma Driver’s license.
  • High School Diploma or GED required.
  • Two or more years of successful sales and/or marketing experience, preferably in the aging services or healthcare environment.
  • Working knowledge of MS Office (Excel, Word and PowerPoint), database management and statistical reporting.
  • Excellent presentation and written communications skills.
  • Self-starter with a high degree of resourcefulness, organized and detail oriented.
  • Possess strong project management skills; and a proven ability to meet deadlines and achieve goals.
  • Positive attitude and professional appearance.
  • High capacity to work independently and with a team.
  • Ability to travel within community, a licensed driver with automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Requires constant hand grasp and finger dexterity; frequent sitting, standing, walking and repetitive leg and arm movements, occasional bending, reaching forward and overhead, squatting and kneeling.
  • Ability to communicate verbally, with an excellent comprehension of the English language.
  • Has access to and ability to drive one’s own vehicle.

Nice To Haves

  • Some college preferred.
  • An understanding of Medicare/Medicaid certification process preferred.

Responsibilities

  • Evaluates potential enrollees to the program to determine participant needs and eligibility for enrollment. Assessments will be most often completed in the home of the potential enrollee.
  • Receives intake telephone calls, explains the PACE program, screens for eligibility criteria, completes inquiry entry into the electronic record and schedules home visits and appointments.
  • Collects basic demographic, financial, emergency contact and service history from potential enrollee.
  • Presents potential enrollee to the team for continued enrollment processing, and assists with coordinating communication as needed.
  • Establishes and maintains an effective working relationship with DHS and OHCA representatives to provide a streamlined enrollment application process.
  • Maintains participant confidentiality at all times.
  • Demonstrates respect for participants and staff.
  • Maintain a cooperative and supportive work relationship with participants, family and staff.
  • Responsible for initiating Medicaid applications and obtaining verification information for completion of Medicaid process.
  • Communicates with referral sources including physician offices, hospital discharge planning, and other community agencies for follow up in completion of the intake record.
  • Provides follow-up communication and correspondence to complete the enrollment process.
  • Maintains documentation of enrollment process, including prospective participants within designed software programs as necessary to complete mandatory state statistical reporting functions.
  • In collaboration with the Senior Intake and Marketing Manager, develop and implement an effective sales and marketing plan.
  • Cultivate strong relationships with key referral sources (Vintage Housing, low-income properties, discharge planners, case managers, medical staff, caregivers, etc.).
  • Consistently strive to locate and cultivate new sources of referral.
  • Ensures prompt communication with each new inquiry and prompt follow-up for each inquiry that is ready to move to the intake and enrollment process.
  • Research, schedule and successfully execute regular community outreach events including health fairs, senior expos, speaking engagements, in-service education and any other event/speaking engagement that would promote LIFE PACE as well as other LIFE branded programs and services.
  • Demonstrates a thorough understanding of HIPAA/HITECH laws and the related policies and procedures within the agency.
  • Attends and successfully completes mandatory HIPAA/HITECH training at least annually.
  • Participate in projects or other duties as assigned.
