About The Position

Salt Lake Behavioral Health is currently seeking a compassionate and talented Intake Assessment Coordinator (RN) to join our team. This is a PRN position, assisting with shift coverage on Wednesday, Thursday, Friday, and Saturday from 6 am to 6 pm. Salt Lake Behavioral Health Hospital is dedicated to transforming lives through exceptional patient care for individuals and families affected by depression, anxiety, trauma, addiction, and suicide. As a private 118-bed freestanding psychiatric hospital, we specialize in mental health and substance abuse treatment for adolescents and adults in the Salt Lake City area, utilizing evidence-based treatments in a comfortable, confidential atmosphere. We believe in an individualized, patient-focused approach to diagnosis and treatment, recognizing that conditions like anxiety or drug dependence can manifest differently in each person, and we are equipped to treat dual-diagnosis cases effectively. The Assessment Coordinator is a frontline clinical decision-maker responsible for conducting comprehensive psychiatric and nursing assessments, coordinating with physicians, utilization review, business office, and external referral sources, and facilitating admissions, transfers, or community referrals across all levels of care.

Requirements

  • Graduate from an accredited school of professional nursing required.
  • Bachelor's Degree.
  • 1 to 3 years experience in Behavioral Health and/or Admissions.
  • Currently licensed to practice by the Utah State Board of Nursing.
  • Thorough knowledge of Joint Commission, HCFA, OSHA requirements, and state and federal regulatory requirements.
  • CPR certification.
  • Successful completion of training in Handle With Care within 90 days of employment, and prior to assisting in restraining procedures.

Responsibilities

  • Providing comprehensive psychiatric and nursing assessments for individuals seeking services.
  • Ensuring timely, accurate, and clinically appropriate admission and referral decisions.
  • Conducting psychosocial and/or nursing assessments.
  • Coordinating with physicians, utilization review, business office, and external referral sources.
  • Facilitating admissions, transfers, or community referrals across all levels of care.
  • Working at least 4 (12 hour) shifts or 48 hours a month.
  • Available to work 2 weekend or evening shifts of at least 12 hours per shift.
  • Work all units as needed.
  • Work 1 major, 1 minor holiday, and 1 hard to staff day.
