About The Position

This is an existing vacancy for a Full-Time Permanent Intake and Resource Coordinator position that performs access and service coordination functions that aid clients in accessing mental health services, and provide resource coordination such as service planning and navigation, for children and youth experiencing complex and multiple mental health concerns, and their families to enable timely and seamless connection to mental health services.

Requirements

  • Diploma, or Degree in a human service field such as social work, child and youth care.
  • 2 years’ experience working in infant, child and youth mental health services.
  • Strong case conception and writing skills to effectively communication assessment details and service recommendations.
  • Knowledge of the infant, child and adolescent development, family systems issues, mental health and addictions, trauma focused approach, harm reduction approach, anti-oppressive practice and working with youth from diverse communities including the LGBTQ+ community
  • Trained in CPI/ crisis management training
  • A Vulnerable Sector police check or Broad Record Check

Nice To Haves

  • Completion of ASSIST training, or similar would be an asset.
  • Lived experience of racism, ableism, homophobia or transphobia, genderism is an asset.
  • Fluency in another language(s), including sign language.
  • Knowledge of services provided within autism services as well as organization-wide services.
  • Awareness of the role of inclusion, diversity, equity, and accessibility (IDEA) in the workplace and within the community being served by the position.
  • An understanding of anti-black racism, anti-oppressive/anti-racism perspective in the context of the social determinants of health.
  • Ability to recognize and identify situations where incidents of imbalance of power and privilege may occur and can result in possible negative impacts.

Responsibilities

  • Coordinate access to Strides’ mental health services to ensure clients are connected to the most suitable programs based on the child’s/ youth’s mental health needs while maintaining timely communication with partners, collaborating with program managers for complex cases, and adhering to service access procedures and reporting requirements.
  • Identify clients needing resource coordination based on needs related to the social determinants of health and promptly work with the client in a time-limited capacity to connect them to the required supports and services.
  • Review, monitor and manage program waitlists, and sharing program specific waitlist information with relevant managers for program planning and coordination of client access to service according to procedural guidelines.
  • Engage in outreach activities to connect with the community and other service providers for the purposes of information sharing, promotion and increasing knowledge and visibility of Strides’ mental health services.
  • Create and maintain a detailed and accurate client record including client, family, and stakeholder interactions, meeting discussions, consultation, clinical reports, and correspondence in accordance with organizational policies and accreditation and licensing requirements.
  • Maintain a positive working environment, contribute to the organization’s mission, strategic objectives and improvement initiatives, and follow the organizational policies and procedures.
