Coordinator, Intake and Referral
About The Position
The Coordinator, Intake and Referral serves as the primary point of contact for individuals entering the Come to Work program. This role is responsible for managing participant referrals, triaging participants to the appropriate stream, conducting intake and orientation processes, assessing participant needs and suitability, and connecting individuals to appropriate program streams and supports. The coordinator ensures a positive onboarding experience while facilitating access to internal and external resources that support participants in achieving their employment goals. This position also manages program inquiries and provides information and guidance to prospective participants and stakeholders.
Requirements
- Experience with career assessments, employment readiness strategies, and trauma-informed practice.
- Previous experience in intake and assessment preferably in a not-for-profit organization.
- Previous experience handling sensitive client information and adhering to privacy and confidentiality.
- Excellent interpersonal, organizational, communication, and collaboration skills.
- Proficiency in Microsoft Office Suite and virtual platforms.
- Post-secondary degree or diploma in career development, social work, human services, education, or related field.
- 1 year experience in intake/assessment related roles
Nice To Haves
- Working knowledge of Salesforce is an asset.
- Experience in the non-profit sector is an asset.
- Personal or professional experience relating to blindness and sight loss is an asset.
- Mental Health First Aid, safe talk, and or certification in trauma informed care is an asset
Responsibilities
- Receive, review, and process referrals for prospective participants entering the Come to Work program.
- Conduct participant intake assessments to determine eligibility, needs, goals, and appropriate program stream placement.
- Facilitate program orientation sessions to ensure participants understand program expectations, services, and available supports.
- Identify participant needs that may require additional supports and coordinate referrals to internal programs and external community services as appropriate.
- Maintain accurate and timely participant records, intake documentation, and referral information in accordance with organizational policies and procedures.
- Accurately update the CRM system to reflect on your work; report back on outcomes and key deliverables
- Triaging participants for either light touch, or full service supports, connecting them to the appropriate employment interventions
- Managing sensitive and confidential information
- Monitor and manage the Come to Work program inbox, responding to inquiries from participants, partners, employers, and other stakeholders in a timely and professional manner.
- Provide information and guidance regarding program services, eligibility requirements, and participation processes.
- Track and report on intake, referral, and participant engagement activities as required.
- Support continuous improvement of intake and referral processes to enhance participant experience and program effectiveness.
- Perform other related duties as assigned in support of program objectives.
