About The Position

Rocky Mountain Care is looking for a full-time Intake/Admissions Nurse (RN/LPN) to facilitate admissions for incoming Skilled Nursing patients. This role does not involve direct patient care but requires verifying and entering physician orders, identifying the level of care needed, and accurately receiving and giving verbal orders. The nurse will review clinical documentation to determine Skilled Nursing placement and manage after-hours and weekend referrals. Exceptional interpersonal and customer service skills are essential for clear communication with medical providers, referring hospitals, physicians, skilled nursing facilities, patients, and families.

Requirements

Current license as a licensed LPN/RN from the Utah State Board of Nursing
1-year Skilled nursing facility experience preferred
Good verbal and written communication skills
Basic computer knowledge
Ability to effectively problem solve
Excellent customer service skills
Ability to commute/relocate: Woods Cross, UT: Reliably commute or planning to relocate before starting work (Required)

Nice To Haves

Knowledge of Medicare/Medicaid Skilled Nursing placement criteria preferred
Knowledge of Medicare, Medicaid and 10A criteria

Responsibilities
