About The Position
Medical Social Worker – LMSW/LCSW (Inpatient/Outpatient) Supporting Patients Across Acute, Ambulatory & Long-Term Care Settings. This role involves delivering high-quality psychosocial care, engaging in crisis intervention, assisting with care transitions, and serving as a key resource for patient education and advocacy within a multidisciplinary care team. The social worker will support patients and their families in various healthcare settings, including acute care, outpatient, and long-term care.
Requirements
- Master's degree in Social Work from a CSWE-accredited program
- Active Licensed Clinical Social Worker (LCSW) in Missouri OR Licensed in Missouri as an LMSW, or eligible to obtain license within 90 days of hire
- At least 2 years of post-graduate clinical experience (for LCSW candidates)
- Solid understanding of clinical theory and social work practice (for LCSW candidates)
- Research and analytical skills (for LCSW candidates)
- Strong written and verbal communication — including public speaking (for LCSW candidates)
- Ability to work across diverse populations and healthcare teams
- Strong knowledge of social work practice and theory at the MSW level (for LMSW candidates)
- Ability to work collaboratively in an interdisciplinary setting
- Excellent communication and interpersonal skills
Nice To Haves
- Experience in healthcare or mental health settings preferred (for LCSW candidates)
- If applicable, must meet requirements outlined in our Fleet Driving Safety Policy (for LCSW candidates)
- Healthcare or mental health care experience is a plus (for LMSW candidates)
Responsibilities
- Assess and address the psychosocial needs of patients and families.
- Coordinate discharge planning, referrals, and protective services as needed.
- Provide crisis intervention, pre-admission screening, and long-term care support.
- Collaborate as part of a multidisciplinary team to support patient-centered care.
- May participate in program development, professional training, and student supervision.
