About The Position

This position is for an Inpatient Medical Social Worker at Aurora West Allis Medical Center, a part-time role with 20 hours per week. The role involves delivering comprehensive social work support, including psychosocial assessments, identifying patient needs, implementing interventions, follow-up care, and discharge planning. The social worker will collaborate with interdisciplinary teams to coordinate care, advocate for patient-centered care, and educate patients and families on healthcare decisions and resources. Responsibilities also include connecting individuals to community services, managing length of stay, reducing readmissions, and ensuring appropriate levels of care. The role requires maintaining clear communication, thorough documentation, and compliance with standards, as well as contributing to staff education and professional development. A strong understanding of social work principles and their application in a healthcare setting is essential. The work environment is fast-paced with established time constraints, and may involve exposure to hazardous materials and life-threatening diseases, requiring adherence to personal protective equipment guidelines. Weekend, holiday, and evening coverage may be required.

Requirements

  • Master’s degree in social work from a Council on Social Work Education (CSWE) accredited school.
  • 1 year of social work experience in healthcare.
  • Social Work Certification issued by the State of Wisconsin.

Responsibilities

  • Deliver comprehensive social work support by conducting psychosocial assessments, identifying patient needs, implementing appropriate interventions, follow-up care, and discharge planning.
  • Collaborate with interdisciplinary teams to coordinate safe, timely care across the continuum, including post-acute services and community resources, while advocating for patient-centered care.
  • Educate patients and families on healthcare decisions, available resources, and complex issues such as advanced directives, end-of-life care, and social concerns.
  • Connect individuals to community services and agencies, ensuring continuity of care and smooth transitions.
  • Support care coordination by managing length of stay, reducing readmissions, and ensuring appropriate levels of care within network systems.
  • Maintain clear communication, thorough documentation, and compliance with regulatory and professional standards, while also contributing to staff education and professional development.
  • Demonstrate knowledge of social work principles and methods and the ability to apply these in a health care setting.
