About The Position

Provide comprehensive social work services, including assessments, care planning, interventions, and discharge coordination, to meet patients’ psychosocial needs. Collaborates with multidisciplinary teams to ensure safe, timely care transitions and access to services such as post-acute care, mental health support, and community resources. Educates and advocates for patients and families, connects them with appropriate services, and addresses issues like end-of-life planning, abuse, and social needs. Manages care progression to optimize outcomes, reduce readmissions, and ensure appropriate levels of care. Maintain documentation, compliance with regulations, and professional standards, while supporting staff training and continuing education.

Requirements

• Masters of Social Work services from an accredited school
• 1-2 years of experience in a clinical, hospital setting post licensure preferred.
• License/Registration/Certification: IL locations-LSW Eligible, LCSW preferred issued by the State of Illinois
• WI locations -Social Work Certification issued by the State of Wisconsin
