Join our team as a compassionate and skilled Inpatient Medical Social Worker. In this role, you will provide psychosocial support to patients and their families while assisting with discharge planning, crisis intervention, and care coordination across a variety of medical and surgical settings, including transplant, pediatrics, and outpatient services. You will complete biopsychosocial assessments, identify patient needs, and develop individualized care and discharge plans in collaboration with the interdisciplinary healthcare team. You will also connect patients with community resources, including home care, skilled nursing, and acute rehabilitation, and provide support during complex situations involving abuse or neglect, substance use, housing instability, and other barriers to care. As a rover, you will travel between Nebraska Medicine locations throughout the Omaha area, including hospitals and outpatient clinics, to provide coverage where it is most needed. This position is a great fit for someone who is adaptable, collaborative, and comfortable supporting a variety of patient populations and care settings. Position Details: Full-time position with a primary focus on weekday coverage Typical shift: 8:00 a.m. to 4:30 p.m. Travel between Nebraska Medicine locations throughout the Omaha area is required Provide support and intervention to ensure assistance for patients and families with personal and/or environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care.
