Social Worker, Inpatient Rehab
About The Position
Under the supervision of a manager, MSW, or licensed clinician, the Social Worker 1 is responsible for providing psychosocial-emotional support and services to patients and families. Assists patient and family in understanding, coping and adapting to the personal, emotional, and bio-psychosocial issues. Provides comprehensive assessments and referrals, crisis intervention, and care management. Collaborates with multi-disciplinary team and other internal and external personnel to promote an awareness of environmental and psychosocial needs and to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community.
Requirements
- Bachelor's Degree in Social Science, Social Work, or related field required
- 1 year of relevant experience required
- Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
