About The Position

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and motivated Registered Nurse to join its Inpatient Care Center team in West Palm Beach. The RN coordinates and delivers compassionate end-of-life care, collaborating with the primary physician and interdisciplinary group to support patients and families. The position is PRN, with a 7:00 a.m. to 7:30 p.m. shift and weekend availability. Trustbridge Hospice serves families across Palm Beach and Broward counties, providing comfort, dignity, and peace to people facing advanced illness. Empath Health is a not-for-profit healthcare organization providing Full Life Care through a connected network of services across Florida.

Requirements

Currently licensed as a Registered Nurse in the State of Florida.
Current Basic Life Support certification.
Understanding of and ability to apply hospice nursing characteristics, including case management, crisis intervention, communication, teaching, thanatology skills, and needs.
Adequate and reliable transportation, adequate insurance, and a valid Florida driver’s license.

Nice To Haves

One year of experience in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing, or hospice nursing.
Basic computer skills and experience with EMR software.

Responsibilities

Coordinate patient and family care in collaboration with the primary physician and interdisciplinary team (IDG).
Conduct thorough assessments of patient and family needs.
Ensure timely and coordinated delivery of interventions and services outlined in the care plan.
Monitor patient conditions, provide interventions, and deliver education to enhance comfort and quality of life.
Address concerns promptly and thoughtfully to support patient and family satisfaction and trust.
