About The Position
Altus Hospice is seeking a Per Diem Hospice RN Case Manager to provide end-of-life nursing care throughout the Kingwood community. The role delivers hands-on hospice care, coordinates services, and supports patients and families throughout the hospice journey as part of an interdisciplinary care team. Care is grounded in compassion, clinical excellence, and connection, with an emphasis on patient comfort, quality of life, dignity, and individualized care planning. The position also shares call duties.
Requirements
- Hold a current, unencumbered Registered Nurse license.
- Have at least one year of RN experience in a medical-surgical or acute-care setting.
- Be proficient with computers, typing, and various programs, including electronic medical records (EMR).
- Maintain current CPR certification if required by the state.
- Have a current driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities
- Respond to clinical referral information and address inquiries and requests.
- Partner with the Clinical Director to assess client eligibility and suitability for hospice services.
- Provide direct nursing care and support to patients, prioritizing comfort and quality of life.
- Deliver nursing care using established principles and techniques to help ease pain and provide comfort.
- Educate and advocate for patients and families, encouraging participation in personalized care plans.
- Share responsibility for call duties.
