This position is for an Inpatient/ER Care Coordinator Licensed Practical Nurse (LPN) whose responsibility is to serve as a key member of the interdisciplinary care team, providing patient-centered care coordination and navigation services across both inpatient and emergency department settings in accordance with accreditation standards, federal, state, and local regulations, organizational and departmental policies and procedures and professional standards. This position is responsible for identifying patients at risk for poor health outcomes, avoidable emergency department utilization, hospital readmissions, and barriers to accessing healthcare services. The Care Coordinator works collaboratively with providers, nursing staff, case management, social services, community agencies, and outpatient partners to ensure seamless transitions of care and improved patient outcomes. The IP/ER Care Coordinator focuses on connecting patients with primary care providers, specialty services, medication assistance programs, financial resources, Medicaid enrollment support, transportation assistance, and community-based services. The coordinator provides post-discharge follow-up and ongoing support to improve continuity of care, reduce healthcare disparities, and address social determinants of health affecting rural populations.
