About The Position

Hospice of South Texas (HOST) is seeking an experienced and motivated Registered Nurse to join their team as an Inpatient Clinical Manager / Clinical Triage RN. This leadership position provides day-to-day clinical oversight of the inpatient unit while supporting staff, coordinating patient care, managing staffing needs, and helping ensure consistent, high-quality hospice care. The position also serves as HOST's RN-level clinical triage resource during assigned coverage hours. This role is for a strong clinical leader who enjoys developing staff, solving problems, and making a meaningful difference for patients and families.

Requirements

Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Current Registered Nurse license.
Minimum of five years of RN clinical experience.
Minimum of two years of supervisory, charge nurse, or clinical leadership experience.
Strong clinical judgment and critical-thinking skills.
Demonstrated ability to lead clinical staff, manage schedules and staffing coverage, establish priorities, and address day-to-day operational needs.
Strong interpersonal, organizational, and communication skills.
Self-directed, dependable, accountable, and able to make timely decisions in a fast-paced clinical environment.
Reliable transportation, current Texas Driver's License, and valid auto liability insurance.

Nice To Haves

Hospice or inpatient nursing experience.
Experience or demonstrated competency in hospice and end-of-life care.

Responsibilities

Provide day-to-day leadership and oversight of inpatient clinical operations.
Manage IPC staffing schedules, assignments, shift coverage, and clinical workflow.
Provide visible leadership and support to both day and night shift teams.
Serve as charge nurse when assigned and provide direct RN shift coverage when necessary to address staffing needs.
Monitor patient acuity, changes in condition, workloads, handoffs, and clinical concerns.
Coach, mentor, and support nursing staff through orientation, competency development, performance feedback, and ongoing education.
Promote high standards for patient care, documentation, communication, teamwork, attendance, and patient/family service.
Collaborate closely with physicians, the Medical Director, Nurse Practitioners, pharmacy, and interdisciplinary team members.
Monitor IPC clinical quality measures, audits, incidents, documentation findings, medication trends, and other identified quality indicators.
Participate in Quality/QAPI activities and follow through on identified opportunities for improvement.
Manage and coordinate HOST's inpatient respite program, including admissions, stays, discharges, staffing readiness, and communication with families and the interdisciplinary team.
Serve as HOST's RN-level clinical triage resource during assigned coverage hours.
Reconcile after-hours clinical activity and ensure unresolved patient needs are appropriately communicated and handed off for follow-up.
Track triage activity and trends and provide monthly reporting to the CNO.
Promote compliance with HOST policies, hospice standards, infection-control practices, safety requirements, and survey readiness.
Identify and escalate significant patient-safety, staffing, regulatory, or operational concerns to the CNO.
