About The Position

The Clinical Triage Specialist serves as the frontline clinical support resource for patients, caregivers, providers, and members of the care team. This role functions as the primary communication bridge between patients and nursing staff by receiving, documenting, prioritizing, and routing patient calls, messages, and care-related requests. The Clinical Triage Specialist supports efficient patient communication, protects nursing capacity, improves response times, and ensures that patient concerns are directed to the appropriate care team member. Working within established protocols and scope of practice, the Clinical Triage Specialist manages administrative and routine clinical support requests while escalating concerns requiring nursing assessment, clinical judgment, or intervention. This position plays a critical role in supporting patient access, continuity of care, operational efficiency, and a positive patient experience. The Clinical Triage Specialist shall not independently assess, diagnose, interpret symptoms, provide medical advice, alter treatment plans, or make clinical decisions requiring professional nursing or provider judgment.

  • Proficiency in electronic health records (EHR), Microsoft Office applications, patient engagement platforms, telephony systems, and electronic communication tools.
