Inpatient Case Manager/ Registered Nurse (RN)- Full Time Weekend Dayshift
About The Position
Sentara Norfolk General Hospital is hiring an Inpatient Case Manager/ Registered Nurse (RN) for a Full-Time Weekend Dayshift position. This role is responsible for the provision and facilitation of comprehensive care management, services, and quality outcomes for patients across the healthcare continuum. The primary focus is on patient transition to the appropriate level of care while facilitating clinical appropriateness and Length of Stay (LOS). This position directly supports hospital patients identified as needing comprehensive case management, promoting effective utilization and monitoring of health services. The Inpatient Case Manager RN collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions, and develops/implements comprehensive care plans based on assessment and evaluation to include physical, mental, and psychosocial needs of the inpatient/caregiver. This role functions within hospital settings.
Requirements
- Registered Nurse (RN) in VA or NC or Compact/Multi-state REQUIRED
- BLS within 90 days of hire.
- 1 year of nursing experience
- Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification is required within one year of eligibility.
Nice To Haves
- 1 year acute care case management preferred
- Bachelor's Degree in Nursing preferred
