Inpatient Case Manager/ Registered Nurse (RN)- Full Time Weekend Dayshift
About The Position
Sentara Norfolk General Hospital is hiring an Inpatient Case Manager/ Registered Nurse (RN) for a Full-Time Weekend Dayshift position. This role is responsible and accountable for the provision and facilitation of comprehensive care management, services, and quality outcomes for patients across the healthcare continuum. The primary focus is on patient transition to the appropriate level of care while facilitating clinical appropriateness and Length of Stay (LOS). The Inpatient Case Manager RN will directly support hospital patients identified as needing comprehensive case management, promote effective utilization and monitoring of health services, and collaborate and communicate with the healthcare team and patient/caregiver to manage care and transitions. A comprehensive care plan will be developed and/or implemented based on assessment and evaluation, including the physical, mental, and psychosocial needs of the inpatient/caregiver. This position functions within hospitals.
Requirements
- Registered Nurse (RN) in VA or NC or Compact/Multi-state REQUIRED
- BLS within 90 days of hire.
- Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification is required within one year of eligibility.
- 1 year of nursing experience
Nice To Haves
- Bachelor's Degree in Nursing preferred
- 1 year acute care case management preferred
