As an Initial Service Plan (ISP) Writer/Service Coordination (SC) Supervisor, you'll play a critical role in helping individuals enrolled in New York's NHTD and TBI Waiver Programs build the foundation for a successful future. This isn't traditional case management. You'll become the expert responsible for developing thoughtful, person-centered Initial Service Plans that open the door to life-changing services. Working alongside participants, families, healthcare providers, and community partners, you'll create individualized plans that empower people to live safely and successfully in their communities. Requirements

  • Meet New York State Service Coordination qualification requirements.
  • Qualifying Master's degree or professional licensure (LMSW, LCSW, RN, PT, OT, SLP, CRC, etc.), OR Bachelor's degree plus three years of qualifying Service Coordination/Case Management experience, OR Associate's degree plus five years of qualifying experience.
  • Valid New York State driver's license and reliable transportation.
  • Experience supporting individuals with disabilities, older adults, or complex medical needs.
  • Passion for helping individuals live independently

  • Strong communication and relationship-building abilities
  • Ability to manage multiple priorities
  • Collaborative problem-solving mindset
  • Knowledge of community resources and disability services

Responsibilities

  • Develop comprehensive Initial Service Plans (ISPs) for participants entering the NHTD and TBI Waiver Programs.
  • Manage a caseload of approximately 25 participants throughout the Rochester region.
  • Build collaborative relationships with participants, families, hospitals, rehabilitation facilities, providers, and community organizations.
  • Conduct assessments and gather information to create person-centered, strengths-based service plans.
  • Coordinate successful transitions from Initial Service Coordination to Long-Term Service Coordination.
  • Serve as a subject matter expert and resource for the Service Coordination team.
  • Assist leadership with mentoring, training, quality assurance, and process improvement initiatives.
  • Partner with leadership to solve complex participant situations and improve service delivery.
