About The Position

Provide individualized coaching, support, and coordination to participants of the Project FORWARD program. Conduct initial and ongoing assessments to identify each participant's needs, strengths, goals, and resources. Maintain effective communication with participants and other service providers to coordinate program activities and ensure Individual Development Plan goals are achieved. Flexibility required for potential nights & weekends to accommodate outreach/workshops/intakes etc.

Requirements

  • Bachelor's degree in human services, social work, mental health counseling, or related field
  • Experience in a mental health related field
  • Experience in coordination of services
  • Experience in employment/education services
  • Experience completing collateral contacts

Nice To Haves

  • Master's Degree preferred
  • Bilingual in English and Spanish preferred

Responsibilities

  • Conduct outreach to recruit/engage potential participants & community organizations serving the target population, presentations to community providers and government entities.
  • Conduct intakes for potential participants in cooperation with Facilitator both in the office and in the community.
  • Conduct initial and ongoing assessments of participants to determine strengths, needs, goals, and resources.
  • Establish and maintain a relationship with participants and serve as a consistent point of contact with them through their participation in the program.
  • Create and monitor each participant's Individual Development Plan to ensure all goals are achieved, and document progress as needed.
  • Assist with referrals and provide follow up to make sure participants are linked to internal and external resources to best meet their needs.
  • Maintain consistent and professional contact with team members and collateral contacts to ensure coordination of care and services.
  • Assist Evaluation Team with research elements of the program and data collection in relation to the local and federal evaluations, including nFORM surveys.
  • Complete program documentation of participants' progress in both participant record and electronic health records.
  • Conduct relationship workshops using evidence-based curricula. Provide support to participants around workshop content and any barriers to engagement as needed both in the office and in the community.
  • Participate in team meetings as scheduled.
  • Support Data Manager with CQI tasks related to the program, including: participant data progress tracking, data sharing with the individuals to whom it applies to facilitate partnered decision-making & progress updates, work with the entire CN Guidance team to apply the Plan-Do-Study-ACT continuous quality improvement approach to address systemic, organizational challenges, complete QI Training during onboarding and participate in QI refresher training as needed.
