About The Position

The Indigenous planning Coordinator supports a safe coordinated and culturally appropriate discharge planning for Indigenous patients and families within Shared Health. The role will be in collaboration with patients’ families, care teams, Indigenous Resource workers and community partners to support smooth transition to urban, rural, and northers situated communities, while encompassing trauma informed and culturally safe and that is patient-centered. The position is responsible for responding to and triaging inquiries from patients, visitors, staff, and stakeholders; maintaining accurate records and databases; and assisting with reporting, correspondence, and presentation materials. The Administrative Assistant plays a key role in supporting referral processes, facilitating communication with external agencies, and ensuring timely information flow to program leadership. This position contributes to building a culturally safe, respectful and trauma-informed workplace, while upholding Indigenous ways of knowing, being and doing, as well as recognizing the impacts of intergenerational trauma and inequities in access to health for Indigenous peoples. THIS IS A DESIGNATED INDIGENOUS COMPETITION, OPEN TO INDIGENOUS APPLICANTS. Applicants are required to self-declare and those selected for an interview will be asked interview questions related to the designation.

Requirements

  • Minimum of 2 years recent experience working with Indigenous patients in an acute care setting.
  • Extensive experience working in and with the Indigenous community and be familiar with available resources both locally and in rural locations.
  • Knowledge of the First Nation and Inuit Health Branch policies and procedures; functions and roles of Nursing Stations; First Nation Health Centres; Indigenous Services Canada, etc.…; and related communications technology as it pertains to patient communication (i.e., telehealth, MS teams, etc.).
  • Lived experience and understanding of Indigenous healthcare priorities, services, and challenges alongside Indigenous culture, protocols, traditions, and values, both within communities and organizations in Manitoba.
  • Lived experience and understanding of Indigenous healthcare priorities and practices, including First Nations, Inuit and Métis cultures, protocols, traditions, values, and rights to self-determination.
  • Familiarity in implementing a multi-disciplinary team approach to client care.
  • Demonstrated experience in navigating multi-jurisdictional levels of government.
  • Bachelor’s degree in a health or social services discipline.
  • Knowledge and awareness of Indigenous culture and traditional practices.
  • Completion of Indigenous cultural safety and anti-training.
  • Thorough knowledge of Windows based programs (Microsoft Word, Excel, PowerPoint, Outlook) and the Internet.
  • Strong and culturally relevant communication, interpersonal, analytical, organizational, and leadership skills.
  • Demonstrated knowledge of Canadian colonial impacts on Indigenous people in social and health contexts.
  • Knowledge and appreciation of the unique history, cultures, and rights of Indigenous Peoples in Canada is required.
  • Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada as per Truth and Reconciliation Calls to Action (2015) and Manitoba’s Path to Reconciliation Act (2016).
  • Understanding and commitment to the articles set out in the United Nations Declaration on the Rights of Indigenous Peoples (UNDRIP) as a framework for truth and reconciliation.
  • Knowledge of Indigenous cultural safety and anti-racism actions, inquests and reports, e.g., Jordan’s Principle, Joyce’s Principle and Brian Sinclair Inquest.
  • Demonstrated knowledge of ethical principles and cultural protocols in interactions with Indigenous partners and organizations.
  • Knowledge or familiarity with Indigenous health issues, population health, and health status of citizens in Manitoba.
  • Knowledge of the Manitoba health care system, federal health care system, provincial and federal governance, and leadership models.
  • Working knowledge of, or experience working with, Indigenous governments, partners, organizations and communities at a service level and structures within Manitoba.
  • Must be able to stand for extended periods of time.
  • Must be able to walk extended distances.
  • May be required to sit / work at a computer for extended periods of time.

Nice To Haves

  • Experience/knowledge of acute, primary and long-term care systems & processes is desirable.
  • Knowledge of Indigenous languages is considered an asset.
