The Independent Living Transition Specialist is responsible for providing core independent living skills for youth aged 16 through 22 that are in foster care or aging out of the foster care system, delivered through a trauma informed, wrap-around approach. Key responsibilities include: ongoing assessments of life skills, transition planning with measurable benchmarks, monthly progress monitoring to support recommendations for movement within the Independent Living (IL) service continuum; access and utilization of internal and community resources when appropriate, intensive support for residents to become proficient in all aspects of IL programming. The position provides support in, identifying individualized goals to complete an Individualized Service Plan (ISP) for each resident to be reviewed quarterly, in the areas of: behavioral health & wellness, finances, nutrition, meal preparation, employment, education, vocation and support in securing permanent housing, Intensive supervision and guidance are critical adjunct skills to ensure all key components of independent living are achieved.
