About The Position

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering programs throughout Arizona, including residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services. CBI provides care to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness, using evidence-based models for prevention, education, and treatment.

The F/ACT Independent Living Specialist works with clients with serious mental illness (SMI) in their homes or in the field to develop independent living skills. These skills may include food shopping, learning bus routes, cleaning the home, maintaining proper hygiene, maintaining relationships with supportive family, and developing community-based supports. The role is expected to spend at least 80% of the time in the field with clients, helping them build skills that support ongoing recovery. The position also provides after-hours and on-call support.

Requirements

• High school diploma or GED.
• Current Arizona driver's license in good standing.
• Clear 39-month Motor Vehicle Record.
• Fingerprint Clearance Card required.
• Behavioral Health Technician (BHT) status in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures.
• Certified Psychiatric Rehabilitation Practitioner (CPRP) certification within 12 months of employment.

Nice To Haves

• Associate degree or higher preferred.
• One year of experience working with the SMI population, with additional experience in multiple settings and levels of care, including mental health and substance abuse, preferred.
• One year of experience in a behavioral health position providing outreach and engagement activities preferred.
• At least six months of recovery from substance use and/or mental health disorders preferred.
• Peer Certification, if applicable.

Responsibilities

• Work with SMI clients in their homes or in the field to develop independent living skills.
• Help clients with activities such as food shopping, learning bus routes, cleaning their homes, proper hygiene, maintaining supportive family relationships, and developing community-based supports.
• Spend at least 80% of working time in the field assisting clients in developing skills that support ongoing recovery.
• Provide after-hours and on-call support.
