The In-Reach Housing Case Manager is responsible for facilitating housing placements, promoting housing sustainability, and supporting participant engagement and socialization within the community for individuals placed through our partner shelter agencies. This role centers on securing stable housing, fostering sustainability, and actively engaging participants to support their transition toward self-sufficiency as they prepare to exit housing subsidies. The ideal candidate is a creative problem-solver who excels at building rapport, staying organized, and motivating others. They will use assertive engagement strategies to help participants remain focused and make progress toward their housing stability goals. The Transition Projects Case Management program covers three distinct areas of participant service: Access, Housing and Retention. Employees hired as Case Managers are expected to be able and willing to perform the duties of each of these functions and may have duty priorities adjusted as needed. Duties may take place at any of our service sites or off premises.
