About The Position

Saint Luke’s Crittenton Children’s Center is seeking a remote In-Home Services Case Manager to join its Pediatric Behavioral Health Team. The position provides clinical case management to children ages 4–18 and their families in their homes and communities in Clay, Platte, and Ray counties, through a contract with Beacon Mental Health. The role is remote but requires on-site attendance for team meetings and work in patients’ homes and community settings. The case manager supports patients throughout their treatment plans, seeing approximately 12–15 families per week. The role coordinates with care providers, schools, families, and outside services to help patients remain at home rather than require hospitalization. Services include treatment planning support, education and coaching, skill building, crisis intervention, and consultation with community agencies. Care is individualized, and the position uses a wraparound approach to connect families with resources, including respite services. The role also includes 24/7 on-call availability.
