In-Home Physician Part Time - Sioux City
About The Position
Signify Health, part of CVS Health, is seeking a part-time physician to provide in-home health evaluations in Sioux City and help bridge gaps in care. Physicians engage with health plan members in their homes, build trusted relationships, and provide flexible, comprehensive, personalized evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the person’s health plan. Clinicians use an iPad and an intuitive clinical workflow. The role does not require prescribing medicine, ordering lab tests, or changing a person’s current treatment regimen. The role involves local and in-state travel as needed, virtual visits when needed, and provision of Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening. The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must be able to perform their duties comfortably and safely in these environments.
Requirements
- Active, unrestricted medical license(s) in the coverage area(s).
- Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
