About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner to provide in-home health evaluations and help bridge gaps in care. The role focuses on building trusted relationships and delivering flexible, comprehensive, and personalized health evaluations in members’ homes. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the person’s health plan. The clinician uses an iPad and a clinical workflow and is not required to prescribe medication, order lab tests, or change a member’s current treatment regimen. Virtual visits may also be conducted as needed. The primary workplace is members’ private residences, which may include household pets, secondhand smoke, or other airborne allergens or irritants. The role requires local travel and travel within licensed states as needed.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic and preventive services when indicated, such as spirometry and diabetic eye exams.
• Use an iPad and the clinical workflow for each evaluation.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Provide Signify Health services as indicated and help connect people with appropriate care for their needs.
• Engage with members, answer health questions, and support improved health outcomes.
