About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner to provide in-home health evaluations and help bridge gaps in care. The role focuses on building trusted relationships and delivering flexible, comprehensive, and personalized health evaluations in members’ homes. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and, when ordered by the member’s health plan, additional diagnostic tests such as diabetic eye exams or spirometry. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen.

The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, or other airborne allergens or irritants. Clinicians must be able to perform their duties safely and comfortably in these settings. The role may also include virtual visits and local travel or travel within licensed states as needed.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.

Responsibilities

• Provide in-home health evaluations and build trusted relationships with health plan members.
