About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner to provide in-home health evaluations and help bridge gaps in care. The role focuses on building trusted relationships with health plan members and delivering flexible, comprehensive, and personalized evaluations in their homes. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and any additional diagnostic tests ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or alter a person’s current treatment regimen. The position involves local travel and travel within licensed states as needed, as well as virtual visits when needed. The clinician’s primary workplace is members’ private residences, which may have household pets, secondhand smoke, or other airborne allergens or irritants. The role includes providing Signify Health services such as diagnostic and preventive services, including spirometry and diabetic eye exams. Full-time employees qualify for a comprehensive benefits package. The stated pay range is $78,331.00–$168,714.00; the actual offer depends on factors including experience, education, and geography.

Responsibilities
