In-Home NP/PA (Full Time) - Trenton, NJ
About The Position
Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The evaluations, lasting about 45 minutes, include a medication and medical history review, a physical evaluation, and potentially diagnostic tests. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medicine, order lab tests, or alter current treatment regimens. The primary workplace is the private residence of health plan members, which can vary in conditions and may include household pets, secondhand smoke, and airborne allergens.

- Board certification (required)

Responsibilities
- Provide In-Home Health Evaluations.
- Engage with people in the comfort of their own homes.
- Bridge gaps in care.
- Build trusted relationships to make people healthier.
- Conduct virtual visits as needed.
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
- Be flexible to travel locally and within licensed states, as needed.
