About The Position

Open Arms Solutions is seeking an experienced, compassionate, and highly organized In-Home Care Coordinator to support clients and caregivers throughout Chicago and the North Suburbs. This role is responsible for managing client relationships, conducting assessments, coordinating care, supporting caregivers, and helping ensure clients receive exceptional service from intake through ongoing care. The ideal candidate thrives in a fast-paced environment, enjoys building relationships, and can effectively balance client service, caregiver support, scheduling coordination, and business development activities.

Requirements

  • Previous experience in home care, healthcare, senior living, case management, social services, or care coordination.
  • Strong interpersonal and relationship-building skills.
  • Excellent organization and time-management abilities.
  • Ability to work independently while collaborating effectively with a team.
  • Comfortable using technology and documentation systems.
  • Valid driver's license and reliable transportation required.

Responsibilities

  • Conduct in-home client assessments and reassessments.
  • Develop and maintain strong relationships with clients and family members.
  • Monitor client satisfaction and address service concerns promptly.
  • Collaborate with scheduling teams to ensure client care needs are met.
  • Support care plan implementation and ongoing service delivery.
  • Serve as a primary point of contact for assigned caregivers.
  • Conduct caregiver check-ins and performance discussions.
  • Support caregiver retention through engagement and proactive communication.
  • Assist with matching caregivers to client needs and preferences.
  • Build relationships with referral partners and community stakeholders.
  • Participate in networking, outreach, and community engagement activities.
  • Support conversion of referrals into active clients.
  • Document referral activity and relationship development efforts.
  • Maintain accurate client and caregiver documentation.
  • Track activities and follow-up tasks in company systems.
  • Partner with recruiting, scheduling, and leadership teams to support growth goals.
  • Ensure compliance with company policies and industry requirements.
