Community Action Partnerships of Western Nebraska (CAPWN) is a non-profit community-based health and human services organization that serves low-income, disadvantaged and those unable to meet their needs through other sources. The agency was established in 1965 and provides services to over 9,000 individuals, children and families in three main areas: Community Health Services, Clinical Health Services, and Supportive Health Services. This dual role Licensed Practical Nurse (LPN) position integrates the responsibilities of Immunization Coordinator and Primary Care Nurse. The LPN provides comprehensive nursing services in both Immunization and Primary Care Clinics, maintaining full responsibility for the management of CAPWN’s Immunization Program and maintaining program compliance under CAPWN and NEDHHS Immunization Program guidelines. The LPN collaborates with nurse and provider teams to support Primary Care Clinic operations, patient education, and health maintenance activities. The LPN delivers direct patient care using a provider-led, team-based Patient-Centered Medical Home (PCMH) model, ensuring services that are accessible, continuous, comprehensive, evidence-based, family-centered, coordinated, compassionate, and culturally effective.
