The ICU Registered Nurse works in a Level III Trauma Center in Puyallup, Washington. The facility has approximately 15 ICU beds and 40 Progressive Care Unit (PCU) beds. The ICU is a high-acuity, adaptable medical-surgical unit and is designated as the health system’s COVID unit, caring for a mix of COVID ICU and medical ICU patients. Patient populations include patients with respiratory failure, GI bleeds, diabetic ketoacidosis, sepsis, complicated postoperative conditions, cerebrovascular accidents, post-arrest conditions, alcohol withdrawal, and intravenous drug abuse. The unit does not provide hypothermia treatment, ECMO, or advanced therapies such as CRRT, Arctic Sun, or Rotoprone. The ICU nurse-to-patient ratio is 1:1 or 1:2 depending on acuity; the PCU ratio is 1:3 or 1:4. Regular floating is expected, including assignments caring for PCU and lower-acuity patients. Unit support includes patient care assistants, a free charge RN and free flex RN to assist with breaks, a 24/7 health unit coordinator, centralized telemetry monitoring, on-site RN, RT, hospitalist, and intensivist coverage 24/7, an ICU pharmacist on the floor from 0700 to 2200, IV therapy support, and a clinical coach/educator. Scheduling is flexible, with block scheduling for night shifts when possible. Schedules are provided three weeks in advance, with notifications sent 10 days before a new schedule begins. Staffing may require six consecutive days. Weekend shifts vary according to unit needs; holidays are assigned based on unit needs. Overtime opportunities may be available when shifts are posted. Day shifts are 0700–1930 and night shifts are 1900–0730.
