About The Position

The ICMS Case Manager provides participant-centered intensive case management, builds ongoing relationships with participants, and coordinates services across Care Campus and community providers to support housing stability, health, wellness, and continuity of care.

Requirements

  • Bachelor’s degree in social work, psychology, sociology, human services, public health, or a related field preferred, unless otherwise required by contract.
  • Experience in case management, homeless services, behavioral health, housing, healthcare navigation, outreach, or related direct services preferred.
  • Experience navigating housing, CES, benefits, healthcare, behavioral health, and community-service systems preferred.
  • Strong participant-engagement, assessment, service-planning, advocacy, documentation, organization, and problem-solving skills.

Responsibilities

  • Complete participant intake, assessment, enrollment, individualized service planning, and ongoing case-management follow-up.
  • Connect participants to housing resources, healthcare, behavioral health, substance-use services, benefits, legal resources, and other supports.
  • Complete CES and other required assessments, referrals, documentation, reassessments, and service coordination in designated systems.
  • Work closely with Client Support Specialists and multidisciplinary partners while maintaining responsibility for formal case-management decisions and documentation.
