This position serves as a Human Services Program Specialist within the Department of Health, focusing on coordinating access to services for clients under the PCPH program. The role involves acting as a patient advocate, linking clients with various community services such as rent assistance, job training, disability claims, transportation, food stamps, Medicaid, and emergency housing. The specialist will research and maintain community resource information, mail resources to clients, and ensure newly enrolled clients receive timely appointments. Additionally, the position coordinates patient specialty care referrals, assists clients with scheduling, follows up to ensure clients are seen by specialists, and obtains necessary medical records. The role also processes PCHP enrollment and disenrollment reports, ensures client engagement with the medical home, and serves as back-up to the Primary Care Senior Clerk. Responsibilities include scheduling and rescheduling appointments, making reminder calls, and contacting no-show clients. The specialist will also perform weekly and monthly record keeping and reporting of data and statistics, and assist in report preparation. The position requires participation in staff meetings, in-service training, and other professional development activities, as well as performing other related duties as assigned.
