About The Position

This is administrative work assisting the Regional Coordinator/Community Program Manager and Human Services Program Specialist in the operation of the CDC funded Florida Breast and Cervical Cancer Early Detection Program (FBCCEDP) at the Hudson location of the Pasco County Health Department. This work involves comprehensive client case management and care coordination, monitoring participant progress. Incumbent serves as a resource for program policies, guidelines, and information. Supports eligible participants in accessing timely breast and cervical cancer screenings and follow-up care. This work includes researching data, gathering data, and data entry. Responsible for monitoring and maintaining client enrollment and care coordination. Uses Microsoft Excel to create and maintain an accurate database for the Florida Breast and Cervical Cancer Early Detection Program, which includes, but is not limited to client information, dates of services, payment of medical claims, and other data as needed. Assists Program Manager in compiling and reviewing program data to complete all required internal departmental reports and reports required by State Health Office in Tallahassee. Reviews client enrollment documents for completeness and accuracy. Reviews and monitors Patient Reporting Forms (PRFs), client eligibility, and tracks missing documentation needed to complete each case. Processes vouchers and referrals. Utilizing evidence-based interventions for complete case management to all enrolled clients. Schedule and/or coordinate screening/diagnostic appointments and referrals. Assists Program Manager and Program Specialist with surgical consultation process, MRI referrals, biopsy referrals and Medicaid application. Responsible for timely and complete client case management and care coordination, following up with clients throughout the continuum of care. Meets regularly with Program Manager and/or Program Specialist to discuss case management and outreach activities. Address barriers (transportation, cost, fear, language). Supports the team to ensure positive and productive workflow. Communicate with clients, providers, and partners by telephone, written communication, e-mail, electronic health records systems (EHR), and fax. Ensures compliance with client confidentiality and program guidelines. Communicates with program providers to educate on program guidelines and assist with care coordination. Processes medical claims for approval/denial and communicates with providers, third party payers, and State Health Office. Monitors incoming medical claims for services and keep track of grant funding expenditures. Updates hyperlinks folder for medical claims processed and denied, reconcile medical claims, and updates missing medical claims list to assure providers are reimbursed for services rendered to FBCCEDP clients. Contact providers and request missing medical claims as needed. Assists with program education and training of program staff, community partners and providers. Travels to other clinics or sites in Pasco, Hernando, Citrus, Sumter counties, and the Tampa Bay area as necessary to facilitate provider education and participate in outreach efforts. Assists in organizing and maintaining outreach materials. Conduct outreach to identify and engage eligible participants. Participates in quality improvement activities. Attend staff meetings and training to promote professional and personal growth. Complete all mandatory training as required by the Program Manager, program, and the agency. Shares telephone duties with other staff, such as receiving telephone calls, keeping accurate messages, and transferring calls to appropriate offices. Processes mail. Greetings visitors, mail delivery, or couriers. Performs other duties as assigned.
